Medical release
Eligibility
Athletes must be intellectually disabled and complete the Special Olympics Kansas medical release that must be signed by a qualified health professional.

Intake Procedure
Applicants should call or mail the office. They will be provided with an registration packet and guidance for contacting a team within their region. On receipt of the completed SOKS Medical Release signed by a qualified physician SOKS will approve individual for participation. Individuals are assigned teams depending upon location.
